Case Overview

A female patient presented with persistent vaginal bleeding (P/V bleeding) after undergoing incomplete abortion managed with D&C.

  • Despite one or two previous dilatation and curettage (D&C) procedures, the patient continued to bleed.

  • Ultrasound (USG) revealed retained products of conception (RPOC) adhered to the uterine wall.

Diagnosis

  • Incomplete abortion with RPOC (Retained Products of Conception)

  • Considered differential: Enhanced Myometrial Vascularity (EMV) / Uterine Arteriovenous Malformations (AVM) – sometimes requiring MRI pelvis for confirmation

Procedure: Hysteroscopic Removal of RPOC

⚕️ Therapeutic Steps:

  • Hysteroscopic visualization of uterine cavity

  • Direct identification of RPOC adherent to uterine wall

  • Complete removal using Hysteroscopic instruments

  • Bipolar resectoscope ablation option available for AV malformations

🎥 A short video of the hysteroscopic procedure was documented for academic sharing.

Execution & Surgical Expertise

👨‍⚕️ Performed by: Dr. Rahul Sangolkar

🏥 Approach: Minimally invasive hysteroscopic management – safer than blind D&C

✅ Advantage: Direct visualization reduces risk of incomplete evacuation & uterine injury

Post-Operative Outcome

✔️ Bleeding stopped completely

✔️ Patient recovered without complications

✔️ No repeat procedures required

Why This Case Was Important

  • Highlights the limitations of blind D&C in managing incomplete abortion

  • Shows the superiority of hysteroscopy in safely removing RPOC

  • Demonstrates how uterine vascular anomalies (EMV, AVM) can also be managed hysteroscopically, avoiding major surgeries
