Rare Case of Unicornuate Uterus with Solitary Fibroid and Single Kidney Successfully Treated at Sangolkar Hospital, Sangli

A 28-year-old female presented with:

  • Persistent left-sided abdominal pain for 1 year

  • Initial Ultrasound (USG) showing:

    • Suspected left ovarian cyst (3 cm)

    • Possible left pelvic kidney

Small ovarian cysts are usually benign and managed conservatively. However, due to persistent symptoms and lack of relief, further evaluation was required.

Advanced Diagnostic Findings

A CT scan revealed unexpected and rare congenital abnormalities:

  • Unicornuate uterus on the right side

  • Absent uterus on the left side

  • Mass on the left side identified as a solitary fibroid arising from undeveloped broad ligament

  • Absence of left kidney (renal agenesis)

  • Only one functional kidney on the right side

Additionally, laparoscopy revealed:

  • A cyst in the undeveloped left fallopian tube, initially mistaken for an ovarian cyst on ultrasound

This case highlights how rare congenital anomalies can mimic common gynecological conditions.

Final Diagnosis

  • Unicornuate uterus (congenital uterine anomaly)

  • Broad ligament fibroid on undeveloped uterine side

  • Left renal agenesis (absent kidney)

  • Fallopian tube cyst misdiagnosed as ovarian cyst

Procedure: Advanced Laparoscopic Fibroid Removal
