A 28-year-old female presented with:


  • Chronic left-sided abdominal pain for 1 year

  • Initial Ultrasound (USG) findings:

    • Suspected left ovarian cyst (3 cm)

    • Probable left pelvic kidney

A CT scan revealed a rare and complex anatomy:


  • Unicornuate uterus on the right side

  • Absence of uterus on the left side

  • Left-sided mass identified as a fibroid arising from an undeveloped broad ligament

  • Left kidney absent (renal agenesis)

  • Only one functional kidney on the right side


👉 The mass seen on ultrasound as a pelvic kidney was actually a fibroid, highlighting diagnostic limitations of USG in complex cases.


Final Diagnosis


  • Unicornuate uterus (Müllerian anomaly)

  • Broad ligament fibroid from rudimentary structure

  • Left renal agenesis (single kidney)

  • Cyst of undeveloped left fallopian tube (misdiagnosed as ovarian cyst)
