High-Risk Infertility Case Successfully Managed with Laparoscopy & Hysteroscopy at Sangolkar Hospital, Sangli

A 34-year-old woman presented with primary infertility and was diagnosed with a septate uterus, a known cause of infertility and recurrent pregnancy loss.

The case was high-risk due to multiple co-morbidities:

  • Epilepsy

  • Obesity

  • Hypothyroidism

These conditions significantly increased the anesthetic and surgical risk.

Diagnosis

  • Septate uterus causing infertility

  • High-risk surgical profile due to neurological, endocrine, and metabolic conditions

Procedure: Laparoscopy & Hysteroscopic Septum Resection

⚕️ Surgical Plan & Execution:

  • Diagnostic laparoscopy to assess pelvic anatomy

  • Hysteroscopic septum resection using a bipolar resectoscope under saline medium

  • Continuous monitoring due to known seizure disorder

Intra-Operative & Post-Operative Challenges

⚠️ Complications Managed Successfully:

  • One episode of convulsions intra-operatively

  • One episode of convulsions on post-operative day 3

  • Continuous multidisciplinary monitoring
