Case History & Premise

A 43-year-old woman from Satara faced an emotionally challenging journey marked by repeated IVF failures—five unsuccessful cycles, to be precise. Despite her determination and numerous attempts at conception, the underlying cause of her infertility remained elusive, leaving her with dwindling hope. The turning point came when a thorough diagnostic evaluation was undertaken to uncover the root issue.

Diagnostic Evaluation

During a detailed laparoscopy, our team discovered that the patient had a bicornuate uterus with a single communicating cavity on the left side. The uterine cavity was notably small and exhibited inadequate blood supply, critical factors that contributed to the failure of embryo implantation. This rare uterine anomaly had been the silent culprit behind her repeated IVF failures.

Innovative Surgical Intervention

With renewed hope, we proceeded with a repeat laparoscopy and hysteroscopy. Using hysteroscopic metroplasty—employing scissors and a bipolar resectoscope—we carefully enlarged the small left uterine cavity. The goal was to create a sufficiently spacious and well-vascularized environment that would support successful implantation in future IVF cycles.

Outcome

The surgical intervention was a resounding success. Post-procedure, we achieved a good-sized uterine cavity, significantly enhancing the prospects for a successful pregnancy. The patient is now on a promising path forward, with renewed hope and improved chances of conception.

Discussion

This case underscores the importance of a comprehensive evaluation in cases of repeated IVF failure. Structural abnormalities, such as a bicornuate uterus, can be subtle yet profoundly affect fertility. Through advanced surgical techniques like hysteroscopic metroplasty, even challenging anatomical anomalies can be corrected, opening the door to new beginnings for patients who once felt that hope was lost.

Conclusion

Every infertility journey is unique, and while repeated IVF failures can be disheartening, innovative interventions and expert care can pave the way for success. Our experience with this case reinforces our commitment to exploring every possible avenue to help our patients realize their dream of parenthood.
